Intensive Case Management Services

Angel Housing holds a Master Agreement with the Los Angeles County Department of Health Services to provide Intensive Case Management Services (ICMS) in Service Planning Area 4. A County partnership to serve highly vulnerable adults experiencing homelessness.

Our case managers do "whatever it takes." They meet people on the street, in interim housing, in clinics, in libraries and parks. Wherever someone is. And stay with them on the long road from crisis to permanent, stable housing.

What we provide

A full circle of support

Through intensive case management, one dedicated case manager connects each person to everything they need to get stable and stay housed.


Outreach & engagement

Meeting people on the street, in parks, libraries, clinics, and interim housing. Building trust before anything else.


Housing navigation & placement

Finding affordable permanent housing, building landlord relationships, and securing subsidies like Section 8.


Move-in support

Coordinating security deposits, furnishings, and utility deposits so a new home is truly ready to live in.


Benefits & income

Enrolling clients in CalFresh, SSI/SSDI, health coverage, and Veteran benefits. Plus budgeting support.


Health & wellness

Linking clients to primary care, mental-health services, and substance-use support, with medication and crisis help.


Employment & education

Job search and placement, vocational training, education, and volunteer opportunities toward self-sufficiency.


Documents & legal support

Recovering vital records. ID, birth certificates, Social Security. Plus eviction-prevention advocacy and referrals.


Daily living & stability

Life-skills and hygiene support, transportation, community connection, and domestic-violence safety planning.

We also fight hunger

Housing and hunger go hand in hand. Alongside case management, Angel Housing provides food to people in need and helps families enroll in CalFresh (SNAP). One of the most effective, dignified ways to keep food on the table.
